How to File an Insurance Claim for a Medical Hair or Cranial Prosthesis
What is a medical wig?
Medical wigs are referred to as “cranial prosthesis” which means that the wig is made especially for people who have medical conditions such has alopecia or cancer. Fashion wigs are worn to change up your look, but medical wigs are designed with the specific wearer in mind.
Step 1 Get pre-approved by your provider. Prior to purchasing a wig, we can provide the estimate of the selected wig or hair piece for your insurance company to find out how much they will cover the costs or reimburse you. Step 2 File an insurance claim. Complete and send Claim Form #1500 to your insurance company. Here’s a list of items you’ll need:
Weave Got the Look Federal Tax ID: #85-2255912
- Procedure Code from your insurance provider for the wig (Codes: A9282 and 99199, commonly used).
- Receipt paid in full as proof of payment with Tax ID and NPI number.
- Rx with your diagnosis code from your doctor. It must be written for a Medical Hair Prosthesis or Cranial Prosthesis, not for a 'wig.'
